This is the order form for the "Full Kit" as required for all U13 and older teams.  The full kit consists of:

2 Short Sleeve Jerseys (1 home and 1 away)
1 Long Sleeve Jersey
1 Pair of Shorts
2 Pairs of Socks
2 Practice Shirts
1 Warm-Up Suit (Jacket and Pants)
1 Soccer Bag

This is the order form for the "Starter Kit" as required for all U12 and younger teams.  The starter kit consists of:

2 Short Sleeve Jerseys (1 home and 1 away)
1 Pair of Shorts
2 Pairs of Socks
2 Practice Shirts
1 Warm-Up Jacket

This is the order form for the "Upgrade Kit" as required for all former U12 players who are now on the U13 teams.  The Upgrade Kit consists of:

1 Long Sleeve Jersey
1 Warm-Up Pants
1 Soccer Bag
